Better Care Fund

The Better Care Fund is a national programme to improve the ways health services and social care services work together. It creates a pooled funding arrangement in local areas to support NHS organisations and the local government to jointly plan and deliver services, with an initial focus on services for older people and people living with long-term conditions.

The Government announced the Better Care Fund in June 2013 and it is seen as a real opportunity for change, to enable people to receive the right care and support, at the right time, in the right place.

Each area has submitted their first plan in 2014 and has been required to submit a refreshed annual plan each year. Our plan has evolved and become more embedded across the local health and social care area.

Better Care Plan

Health and social care integration lies at the heart of Better Care Fund ambitions and is embedded within all of the Bristol, North Somerset and South Gloucestershire wide strategies and plans. We have committed to work together with local authorities across our area to deliver health and care services for our local population.

Our approach is part of an on-going process to transform local health and social care services, with a focus on putting our population at the forefront of our planning to improve the lives of people in our community through joint working and joint commissioning of services so that they have fewer health crises and fewer admissions to hospital.

During the planning process in 2017, the three then separate CCGs within Bristol, North Somerset and South Gloucestershire worked together to align their Better Care Plans, whilst ensuring the key priorities of each of their local populations were still met. This has enabled the development a number of joint arrangements, whilst building on the extensive partnership working that had already been in place.

Our Bristol, North Somerset and South Gloucestershire Better Care Fund plan will strengthen our performance management capacity and the quality of the information. The positive impact of multiple schemes across the whole health and social care system is key to building the momentum for successful change and improving services.

We are currently finalising our Bristol, North Somerset and South Gloucestershire Better Care Fund plan for 2019/20 and this will be published here soon.
