Together, we have set out how health and care services will work in partnership and have developed plans that will enable us to continue to meet the health needs of our local population at a time of increasing demand and constrained resources.
We have collated existing feedback from service users, carers and the public to help inform our planning. This includes information from public engagement activities, local surveys and local health scrutiny committees, and information collated from ‘friends and family’ test data, patient complaints and Care Quality Commission reports.
Our thinking has been shaped by the issues that those who rely on our services have told us are important to them.
This Sustainability and Transformation Partnership Plan Summary describes how we propose to organise and deliver services in a different way to meet the needs of the population across Bristol, North Somerset and South Gloucestershire.
